We there for a kid’s birthday party. The staff was great with the kids and organized various drills and games for them to participate in. There is also plenty of room for guests to sit at picnic style tables and benches and bleachers as well to sit and watch.

Liability Waiver

Northridge Futsal League — Liability Waiver and Release

1. Acknowledgment of Risk

I acknowledge that participating in indoor soccer, basketball, volleyball, gymnastics, and other activities at Northridge Futsal League involves inherent risks of injury, including but not limited to sprains, fractures, concussions, and other serious injuries. I understand that these risks cannot be eliminated regardless of the care taken by the facility.

2. Assumption of Risk

I voluntarily assume all risks associated with my participation in any activities at Northridge Futsal League, including but not limited to the risk of injury from falls, collisions with other participants or objects, equipment failure, and overexertion.

3. Release of Liability

In consideration of being permitted to use the facilities and participate in activities at Northridge Futsal League, I hereby release, waive, discharge, and covenant not to sue Northridge Futsal League, its owners, employees, agents, and affiliates from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained while participating in activities or using the facilities.

4. Indemnification

I agree to indemnify and hold harmless Northridge Futsal League, its owners, employees, and agents from any loss, liability, damage, or costs they may incur due to my participation or the participation of any minor under my guardianship.

5. Medical Treatment

I authorize Northridge Futsal League and its staff to seek emergency medical treatment for me or my minor child in the event of an injury, and I agree to be financially responsible for any medical costs incurred.

6. Minors

If I am signing on behalf of a minor, I represent that I am the parent or legal guardian of the minor and have the authority to enter into this agreement on their behalf. I agree to be bound by all terms on behalf of the minor.

7. Facility Rules

I agree to follow all facility rules and instructions provided by staff. I understand that failure to comply may result in removal from the facility without refund.

8. Photo/Video Release

I grant Northridge Futsal League permission to use photographs and video taken during activities for promotional purposes without compensation.

9. Agreement

By signing below, I acknowledge that I have read this waiver, fully understand its terms, and sign it freely and voluntarily without any inducement.
