WAIVER FORM

In consideration for being permitted to enter the Northridge Futsal, Inc. premises located at 19851 Nordhof PL., Chatsworth, CA913111 for any purpose including but not limited to observation and/or use of its facilities, services, participation in futsal activities such as Soccer, Basketball and volleyball and/or use of its equipment, l, the undersigned, agree, understand, and acknowledge the following:

I herby release , waive, discharge Northridge Futal, Inc, the owners, subsidiaries officers, managers, instructors, employees, teachers, volunteers, agents, owners and lessors of the premises and all other persons or entities acting in any capacity on their behalf (hereinafter referred to as "Northridge Futsal", and all other Participants /individuals at the facility from all liability to me, my personal representatives.

Assigns, heirs, and next of kin from any loss and/or damage, and any claims, demands, causes of action, and/or lawsuit, which are in any way connected to my participation in futsal activity and/or my use of Northridge Futsal services, equipment, facilities, and/or observation/presence thereof, including any such claims which allege Negligent acts and/or omissions and/or misconduct by Northridge Futsal, I, the undersigned, further agree to indemnify, defend, and hold harmless Northridge Futsal for and from any such claims.

I hereby agree to indemnify, defend, save, and hold harmless Northridge Futsal from any loss, liability, damage and/or cost they may incur due to the alleged negligence, intentional act and/or other acts by me, any other individual at the premises, and/or Northridge Futsal.

I understand and acknowledge that Soccer, Basketball and volleyball involves known and unknown/unanticipated risk that could result in physical or emotional injury, paralysis, death, and damage to the named participant/player, to property, and/or to third parties. I understand that such risk simply cannot be eliminated without jeopardizing the essential qualities of the activity. The risks include, but are not limited to, broken bones, sprains, strains, and other soft tissue injuries, bruises, abrasions, lacerations, dental injuries, concussions, spinal cord injuries, and/or death. I expressly agree and promise to accept and assume all of the risks existing in any activity involving participation and/or presence at Northridge Futsal, the services, equipment, and/or facilities thereof. My participation in an activity is purely voluntary, and I elect to participate in spite of the risks.

I understand that wearing safety equipment can reduce the risk of injuries and agree that it is my sole responsibility that I properly wear/use safety equipment when I am participating in any activities using Northridge Futsal services and/or facilities. I understand that i am solely responsible for my personal health and safety and my personal property, and the alleged loss thereof.

I understand that Northridge Fustal has a no tolerance policy as to altercations and/or fighting at their facilities/premises. As such, I assume all risks and liability for any alleged loss, damage, injury, death, and/or loss to property due to any altercations and/or fighting at the Northridge Futsal facilities whether caused by me or others. I further agree to indemnify, defend, and hold Northridge Futsal harmless for such.

I certify that I have adequate insurance to cover any injury or damage I may cause and/or suffer while a participant and/or present at Northridge Futsal, or else i agree to bear the cost of such injury and/or damage myself. I further certify that I have no medical or physical conditions, which could interfere with my safety and/or health in this activity, or else I am willing to assume and bear the cost of all risks that may be created, directly or indirectly, by any such condition.

Should Northridge Futsal or anyone acting on their behalf be required to incur attorney's fees and/or costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.

I understand and agree that this Agreement is intended to be as broad and inclusive as permitted by the laws of the State of California, and that if any portion of it is held invalid, I agree that the remaining terms shall continue to be in full force and effect.

I understand and agree that this Agreement is an important legal document and declare that I have read, considered it carefully, understand, agree, and voluntarily sign this Agreement; and further agree that no oral representation, statement, and/or inducement to sign have been made apart from what is in this Written Agreement.

FOR PARENTS/GUARDIANS OF PARTICIPANT OF MINOR AGE (UNDER AGE 18 AT TIME OF REGISTRATION)

This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liability incidents to my minor child's involvement or participation in these programs as provided above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
